The drug inhibits PDE5, the phosphodiesterase isoenzyme responsible for clearing cyclic GMP out of the smooth muscle that lines the penile vasculature. When that clearance is slowed, cGMP stays where it is, the muscle relaxes and inflow rises. What the mechanism does not do is create a signal: arousal still has to come first, so the tablet amplifies a natural event rather than manufacturing one. Pharmacokinetics are what define the use case. Absorption is fast, with an onset around 20 minutes, and the elimination half-life is 3-4 hours, which produces a practical working window of roughly 4-6 hours from a single tablet; after a 100 mg dose the response has been documented for at least three hours. A heavy fatty meal delays absorption and softens the onset, so timing matters more here than with the long-acting member of the same class, Cialis DP, whose half-life of about 17.5 hours spreads the effect across a day and a half. Sildenafil is cleared largely through CYP3A4, and drugs that inhibit that pathway, erythromycin and cimetidine among them, extend its stay in the body.
